Referral Process

Anyone interested in referring a family or a single female to our program should download an application and submit it to us as soon as possible. This allows us to both pre-screen the applicant for eligibility and to place them on our waiting list.


A. Forms

  1.   Dear Caseworker letter
  2. Referral Application
  3. Coordination of Services
  4. Release of Information
  5. Individual/Family Service Plan (or Treatment Plan)
  6. Other documentation needed to complete the application
    • Proof of homelessness-see HUD's definitions (http://www4.law.cornell.edu/uscode/42/11302.html)
    •   Letter of introduction by the referring case worker
    • Psychosocial (if available)

B. Referral steps

  1. Referral: Once an application is received for a client, the LTHP social worker conducts a preliminary evaluation to assess client eligibility.
  2. Waiting list: Client's who qualify are placed on the waiting list and the referring caseworker is contacted regarding both missing paperwork and client status.
  3.   Interview: The social worker contacts the referring caseworker when the client's name comes up on the waitlist. At this point, an interview is set up with both the referring case worker and client.
  4. Field/Site Visit: The interview is followed up by a home/shelter visit.
  5. Drug Screening: If accepted, clients are asked to take a drug test.
  6. Intake: Clients move in, sign all the necessary forms (including the lease), have their rent calculated, get their keys, etc.

Note: Steps 3-6 can take up to two to three weeks to complete.
